    Sec. 7-33(e), Ordinance: Direct supervision means watching and directing the childrens activities within close proximity within the same room inside or within a designated outdoor play area and responding to each childs needs. Inadequate supervision occurred when a child/childrens needs were not met in the following incident: [Child left in restroom unatended]. Comments: Provider self reported that a child went into a bathroom and was being supervised by a teacher who became distracted and the bathroom door closed. Another teacher assisted when she heard the child in the restroom. FINDINGS: An incident report was created and parent was notified. A statement from the Director was received. Per the director, she stated there was a teacher holding the door open when a child spilled liquid on the floor. She went to attend to the child and clean up the floor. At that point, the restroom door closed, leaving the child alone inside. The child was found by another teacher in the adjacent class when she heard the child crying inside the restroom. The director was asked how long the child was inside the restroom to which she stated that it was less than five minutes. The child could not open the door by herself. The director stated that they have purchased items to keep the door from closing to allow for better supervision while children are in the restroom.
