30. First Aid/CPR Certification Group: DCF Handbook (30-02)

4.2.4(B), Handbook: the facility shall maintain the following number of child care personnel with the CPR certification and first aid training that are on site at the facility at all times children are in care. For child care personnel to count to meet requirement in the below chart, they shall be child care personnel assigned to provide direct supervision of children in care: 1. For 0-25 child care personnel in direct supervision of children to meet staff to child operating ratios there must be 3 child care personnel with pediatric CPR and first aid certification, 2. The youngest child was 2 and the staff to child ratio is 1 staff to 11 children. The Director who was doing task around the facility came outside to stay with the children. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. Sec. 7-4.06(c), Ordinance: There shall be at least one (1) staff person providing direct supervision at all times when one (1) or more children are present. At no time shall any child be left alone for any reason without direct supervision. Direct supervision means watching and directing the childrens activities within close proximity within the same room inside or within a designated outdoor play area and responding to each childs needs. Child care personnel and volunteers at a facility must be assigned to provide care to a specific group of children and be present with that group of children or child at all times during the day including during meals, napping, snack time, and transportation of children.2.4.1(B), Handbook: Children must never be left inside or outside the facility, in a vehicle, or at a field trip location by themselves. Lack of direct supervision was observed in the [see comment]. Ratios/Discipline [SR] Comments: Quality Assurance Specialist arrived at the facility on 10/17/2024 at 2:30 PM to measure and approve a new Toddler classroom. QAS greeted the Director/Owner (EM) who was sitting in the front office. She escorted me down the hallway to measure the new room when QAS observed a Three/Four-year-old classroom with 9 children napping on mats without a staff member. A few minutes later a staff (YH) appeared from the playground area wearing gloves, as she was serving food to children. QAS then walked outside to the playground area and observed 1 staff (YL) with 16 children six of the children were observed to be one-year old's. Children were observed crying walking in and out of the playground door and one toddler was walking down the hallway unattended. Director/Owner stated that some teachers are late coming back from lunch. Staff did not have updated attendance when classrooms were combined. Classroom attendances were provided to QAS from the office. To come into compliance QAS assisted staff provide the correct supervision and staff/child ratios by removing the toddlers to an inside classroom and providing 4 staff with including the Director with 36 children on the outside playground. Technical assistance was provided to the director for children 1 year of age or older, but under 2 years of age, there must always be one childcare personnel for every six children even in mixed groups. Ratios of staff to children and supervision- In groups where children of varying ages are combined, the staff-to-children ratio shall be based on the age of the youngest child in the group. Direct Supervision was not provided in multiple classrooms therefore a Class I Notice of Violation is being issued. The violation occurred on 10/17/2024.
