7-33(d), Ordinance: When children are napping, the child care direct supervision requirements set forth below shall apply in addition to the staffing ratio requirements: (1) If any of the napping children are under 12 months of age, there must be one child care staff member for every four children. (2) If any of the napping children are under 2 years but over 12 months of age, there must be one child care staff member for every six children. (3) If the napping children are all at least two years of age but none are three years of age or older, there must be one child care staff member for every 22 children. (4) If the napping children are all at least three years of age, there must be one child care staff member for every 30 children. During naptime, a staff-to-child ratio of [See commens] staff for [See comments] children was observed. Comments: Per staff member's statement, two (2) children from the one year old classroom that had a ratio of 1:6 were transferred to the adjacent two year old classroom that had a ratio of 2:12 during nap time. Once children were transferred the one year old had a ratio of 1:4 and the two year old classroom had a ratio of 2:14. Ratios were corrected at the time of the inspection, and technical assistance was provided regarding ratios during nap time. After ratios were corrected, the following staff to child ratios were observed: Infants 1:4 Ones 1:4 and 1:5 Twos 2:12 Threes 2:14 Fours/School Age 1:14 and 1:18 08. 2.5.2(A)(1-5), Handbook: A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must include: (1.) Each childs name, (2.) The date and time of departure, (3.) Time of arrival at the destination, (4.) The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and (5.) The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable). The facilitys log did not include all required information. (Section 2.1(A) Health and Safety, Page 3) Transportation [SR] Comments: Transportation log was observed missing the second staff member signature on 1/10/25. Provider does not have any field trips scheduled in the near future, however once the program decides to conduct a field trip prior the next routine inspection, provider needs to submit via email a copy of the transportation log at the end of the trip.
