The personnel/ volunteer (ten hours or more per month) record did not include a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. SACCF Handbook, Section 7.4, C Comments: During the time of inspection, the Operator advised the Licensing Specialist that there 5 employees. During the time of the inspection, the Licensing Specialist monitored 5 of the 5 employee records. The staff noted on the supplemental page did not have the above mentioned document on file. 05. Transportation SACCF Handbook, Section 2.5 (05-04)

The facility's transportation log did not include [the second sweep and the day of the inspection it was missing the drivers and second sweep signature]. SACCF Handbook, Section 2.5.2 Comments: During the inspection, Licensing Specialist observed there was no second sweep completed and no signature. On 10/23/24 the driver and the 2nd personnel signature was missing. The director was given until November 25, 2024, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, SACCF Handbook, Section 2.5.2 Transportation Log A. A log must be maintained for all children being transported in the vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. Date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of traveling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the parent to verify the transportation log and that all children have left the vehicle (if applicable). 06. Driver's License, Physician Certification & First Aid/CPR Training SACCF Handbook, Section 2.5.1 and SACCF Handbook, Section 7.4 Compliance Compliance Comments During the inspection, the director advised that currently there is one transportation vehicles a 2013 Collins Bus (166614) that was last inspected on 9/23/24 and is insured through Philadelphia Insurance Company with an effective date of 1/6/24.
