An area(s) of the facility was observed to not be in good repair. CCF Handbook, Section 3.1, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, the Licensing Specialist observed areas of the facility was observed to not be in good repair in that in the two and three-year-old classroom there was a cabinet with blue doors that was chipped at the corners and along the edges, exposing the brown board underneath. The toilet in the One and Two-year-old classroom bathroom did not have a cover to the tank. The staff informed the Licensing Specialist that the toilet does not flush and that it is being replace. The Licensing Specialist did not observe any children in the classroom. The Operator informed the Licensing Specialist that the classroom is not being used currently due to the toilet not working, and that the children were moved to a different classroom with a working toilet. The light in the One and Two-year-old classroom bathroom did not come on when the Licensing Specialist and a staff member attempted to turn on the light. The Operator has been given until 7/8/2026 to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, CCF Handbook, Section 3.1 A: General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children. Documentation of the indoor play area inspection must be maintained for 12 months. 13. The facilitys outdoor play area contained litter, nails, glass or other hazards that posed a low potential for harm to children. CCF Handbook, Section 3.5, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, the Licensing Specialist observed the facilitys outdoor play area contained hazards that posed a low potential for harm to children. On the Large playground under the yellow cover that is at the highest level of the jungle gym the posts that hold up that yellow cover are rusted and chipping. The four posts are within childrens reach. All areas of playground equipment need to be cleaned in that throughout the large playground, the equipment surfaces contained a black substance. Stored food was not consumed or discarded on or before the expiration date listed by the manufacturer. CCF Handbook, Section 3.9.2, B (Section 2.1 Health and Safety, Page 3) Food and Nutrition [SR] Comments: During the inspection, the Licensing Specialist observed stored food was not consumed or discarded on or before the expiration date listed by the manufacturer. The stored food was two cartons of eggs which expired on 6/7/2026 and 5/13/2026. This standard has been brought back into compliance in that the Operator discarded of the eggs at the time of the inspection. During the inspection, the Licensing Specialist provided technical assistance, CCF Handbook, Section 3.9.2, B: Food Storage: Proper storage of food is essential to prevent food contamination, as well as, insect and rodent infestation. Correct handling and storage of all food is a key component in preventing food-borne illnesses. To prevent bacteria growth, cold food must be kept at or below 41 degrees Fahrenheit and hot foods at or above 135 degrees Fahrenheit. Facilities choosing to prepare food must have a designated space for food storage within the designated food preparation area or in a room not calculated as part of indoor floor space, and in an area not used for diapering. Off-site food storage is permissible only if the site of storage is a licensed childcare facility under the same ownership that includes a food preparation area that meets licensing standards. During the inspection, the Licensing Specialist provided technical assistance, School Readiness Program Health and Safety Standards Handbook: Section 3.1: Preservice: Childcare personnel must successfully complete the preservice training coursework described in this section, as applicable. All childcare personnel must complete these preservice training requirements within 90 days of initial employment with any School Readiness provider. This timeframe does not start over if personnel change employment to another School Readiness provider within this initial 90 days. CCF Handbook, Section 6 (Section 2.1 Health and Safety, Page 3) Health Requirements [SR] Comments: During the inspection, the Licensing Specialist observed that the facility does not have documentation for 8 childcare personnel on required annual refresher education of the facilitys exposure plan. The Operator has been given until 7/8/2026 to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, CCF Handbook, Section 6: Health Requirements: There are three common modes of transmission for the spread of microorganisms in childcare settings: contact, droplet, and airborne. Many common infections encountered in the childcare setting are transmitted by direct or indirect contact. Childcare facilities shall develop a written exposure plan regarding universal safety precautions, recommended by the Centers for Disease Control and Prevention (CDC), to follow in the event there is exposure to blood and potentially infectious fluids. Personnel are required to be educated on the facilitys exposure plan regarding standard precautions before beginning to work and annually thereafter. Written documentation that childcare personnel have participated in the facilitys annual refresher education for the exposure plan and understand the proper procedures in the event there is exposure to blood, and potentially infectious fluids must be retained for 12 months and be available for licensing to review. 36.
