The facility did not have documentation of an annual vehicle inspection for a vehicle used to transport children in care. CCF Handbook, Section 2.5.4, A Comments: During the inspection, the Licensing Specialist observed the facility did not have documentation of an annual vehicle inspection for a vehicle used to transport children in care available for Licensing to review during the inspection. The Operator has been given until 6/13/2026 to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, CCF Handbook, Section 2.5.4, A: Vehicle Requirements: For the purpose of this section, vehicles refer to those owned, operated or regularly used by the childcare program, and vehicles that provide transportation through a contract or agreement with an outside entity. Parents/guardians personal vehicles used for transporting during field trips are excluded from meeting the requirements of this sub-section. A. All vehicles regularly used to transport children must be inspected annually by a mechanic to ensure that they are in proper working order. 13. Toxic Substances, Hazardous Materials and Weapons CCF Handbook, Section 3.2 (13-03)

Harmful items including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials were accessible to children in care. CCF Handbook, Section 3.2, B Comments: During the inspection, the Licensing Specialist observed the facilitys storage of harmful items including cleaning supplies, flammable products, and poisonous, toxic and hazardous materials allowed access by children in care. The cleaning supplies was a bottle labeled bleach water in classroom 170. This standard has been brought back into compliance in that a staff member moved the cleaning supplies out of childrens reach. During the inspection, the Licensing Specialist provided technical assistance, CCF Handbook, Section 3.2, B: Toxic/Hazardous Material/Firearms/Weapons B. All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times. 41. Immunization Records CCF Handbook, Section 7.1 (41-02)

The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)]. CCF Handbook, Section 7.1 Comments: During the inspection, the Licensing Specialist observed 1 Florida Certificate of immunization was not acceptable in that the form was not current (expired). The Operator has been given until 6/13/2026 to come into compliance with this standard. During the inspection, the Licensing Specialist provided technical assistance, CCF Handbook, Section 7.1: Immunization Records: The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C- 22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. A. Immunizations received out-of-state are acceptable; however, immunizations must be documented on the Florida Certification of Immunization form and must be signed by a physician practicing in the State of Florida. B. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. The parent/guardian of a child who has not received the age- appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide documentation of a scheduled appointment or arrangement to receive immunizations. Providers must include a general statement in parent handbook/policies to inform parents/guardians, at time of enrollment, that some children in care may not have current immunizations. C. School-aged children attending public or non-public schools are not required to have student health examination and immunization records on file at the child care facility as such records are on file at the school where the child is enrolled. D. If the custodial parents or legal guardians need assistance concerning these requirements, the facility shall refer them to the Department of Health or to the childs physician. E. Medical records in this section are the property of the custodial parent or legal guardian and must be returned to them when the child withdraws from the facility. The medical records are transferable if the child attends another facility. F. It is recommended that child care personnel are current with all immunizations routinely recommended for adults by the Advisory Committee on Immunization Practices (ACIP) of the Centers for Disease Control and Prevention (CDC), as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by in 65C-22.001(7)(v), F.A.C. 33. Training Requirements CCF Handbook, Section 4 (33-06)

The facility did not have documentation to show completion of a Department approved five-hour early literacy and language development course for child care personnel within 12 months of date of employment in child care industry and/or the early literacy course documentation was not uploaded in the Florida Pathways/Registry. CCF Handbook, Section 4.2.2 and 4.5, C Comments: During the time of inspection, the Owner/Operator/Director advised the Licensing Specialist that there 6 employees. During the time of the inspection, the Licensing Specialist monitored 6 of the 6 employee records. During the inspection, Licensing Specialist updated the People Section in CARES. REMINDER: In-service training for 2024/2025 fiscal year must be completed before July 1, 2025. 33-6: During the inspection, Licensing Specialist observed 1 child care personnel that did not complete the early literacy training within 12 months of their child care industry start date. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 4.2.2 and 4.5, C Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the child care industry. Proof of completion may be documented on a certificate of course completion, classroom transcript, or diploma. Child care personnel must complete one of the following: A. One of the Departments online literacy courses available on the Departments website. B. One of the Departments approved literacy courses. A list of these courses can be obtained from the Departments website. (No additional courses will be approved by the Department.) C. The online CPR training did not include an on-site instructor-based skills assessment and is invalid for the individual. CCF Handbook, Section 4.2.4, E Comments: 36-4: During the inspection, Licensing Specialist observed that there was no employee with a valid CPR card with the skills stated. The director advised that there are 6 employees with a CPR card that did not state that the skills were met. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 4.2.4, E CPR courses must include on- site, instructor- based skill assessments by a certified CPR instructor. Documentation of completion of the online course and on-site assessment must be maintained at the facility and available for review by the licensing authority. CCF Handbook, Section 7.2 Comments: 42-2: During the inspection, Licensing Specialist observed 1 childs file with expired physical. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 7.2 The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. Compliance Comments During the time of the inspection, the Owner/Operator advised the Licensing Specialist that there are currently 42 children enrolled. During the time of the inspection, the Licensing Specialist monitored 42 of the 42 childrens records. REMINDER: Influenza brochures must be provided annually between August 1st and September 30th for each enrolled child. The personnel/ volunteer (ten hours or more per month) record did not include a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. CCF Handbook, Section 7.4, C Comments: During the time of inspection, the Owner/Operator/Director advised the Licensing Specialist that there 6 employees. During the time of the inspection, the Licensing Specialist monitored 6 of the 6 employee records. 44-4: During the inspection, Licensing Specialist observed 4 employees did not sign the child abuse and neglect form annually. During the inspection, staff signed the child abuse and neglect form. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 7.4, C Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: A. A complete employment application with the required statement pursuant to Section 402.3055 (1)(b), F.S. B. Documentation of position and date of employment. C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel. Compliance Comments During the inspection, Licensing Specialist reviewed the background Screening Clearinghouse Roster. Care Provider Background Screening Clearinghouse: The Background Screening Clearinghouse will generate e-mails and send you, the provider, a reminder at 60, 30, and 21 days prior to the retention date expiration. Please note, you will only receive that notification for the staff that are on your facility/home roster.
