    The facility did not have documentation of a fire extinguisher being properly maintained to include being serviced and retagged timely, and/or with a current certificate. SACCF Handbook, Section 3.8.2, B Physical Environment [SR] Comments: At the time of the inspection, the licensing specialist observed the fire extinguishers tagged in the cafeteria where the program is located; did not have current dates of service on them (Dec 2025). The provider stated that as far back as October he reminded the school of the due dates, however they did tell him that they would be serviced but not been completed as of yet. SACCF Handbook, Section 7.4, C Record Keeping [SR] Comments: At the time of the inspection, the licensing specialist reviewed all staff files and observed the staff listed on the supplemental had an expired Child Abuse & Neglect form in their file. Technical assistance was given to the provider to have the staff sign a new form prior to the due date. The provider was reminded this form must be signed annually on or before the prior year's date.
