There were bulbs burned out in the 2-year-old classroom and the lighting only measured 10.18. TA was provided that the lighting is required to be 20 ft candles. CCF Handbook, Section 3.1, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: At the time of the inspection, the licensing specialist observed that the 2-year-old classroom had duct tape holding the cover to the intake closed, the toilet tank cover in the 4-year-old classroom restroom was cracked and sharp, in the VPK restroom the toilet paper holder was broken and taped up with duct tape, the bookshelves in the 4 year old classroom had peeling contact paper and in the VPK classroom had a bookshelves has a few areas of broken laminate and there were several ceiling tiles in the 4 & 5 year old classroom. TA was provided to repair the intake cover, replace the tank cover, repair the bookshelves, replace or repair the toilet paper holder and replace the ceiling tiles by the due date.
