The current year was completed on 9/12/2025 but had violations that needed corrections with a due date of 10/12/25, and the prior year was completed on 9/6/2024. Technical assistance was given to the provider as a reminder that: Prior to transporting children by vehicle, the transportation log must be recorded with each childs name, the date and time of departure and initialed by child care personnel verifying that all children were accounted for and that the log is complete. Upon arrival at the destination by vehicle, the driver of the vehicle must: a. Mark each child off the log as the child departs the vehicle; b. Conduct a physical inspection and visual sweep of the vehicle interior to ensure that no child is left in the vehicle; and c.
