The facility's transportation log did not include [see below]. CCF Handbook, Section 2.5.2 (Section 2.1 Health and Safety, Page 3) General Requirements [SR] Comments: 5-4: During the inspection, Licensing Specialist observed that some fieldtrip transportation logs entries were missing required information, such as checking the children on/off the vehicle, the time on/off the vehicle, drivers signature and secondary signature. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook, Section 2.5.2 A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable). Compliance Comments The director advised that there are 5 drivers. Driver 1s license will expire on 9/16/2027, physical was conducted on 3/19/2025, the CPR and First Aid certificate is valid until 8/30/2026 and the driver has completed the transportation training. Driver 2s license will expire on 4/23/2030, physical was conducted on 11/3/2024, the CPR and First Aid certificate is valid until 8/30/2026 and the driver as completed transportation safety training. Driver 3s license will expire on 10/30/2030, physical was conducted on 9/17/2024, the CPR and First Aid certificate is valid until 9/30/2025 and the driver as completed transportation safety training. 06. The facility did not have a fire extinguisher with a minimum rating of 2A10BC available in the food preparation area. CCF Handbook, Section 3.8.2, B (Section 2.1 - Health and Safety, Page 3) Physical Environment [SR] Comments: The last fire drill was conducted on 7/1/2025. The fire extinguisher certification(s) are valid until 2/2026. The provider advised that their facility was stuck by lightning and the fire panel is non-operational. The fire marshal has placed the facility on a fire watch. The provider advised that the part needed to fix the fire panel is on back order. 23-3: During the inspection, Licensing Specialist observed that the vehicle (A56046) did not have a fire extinguisher with a minimum classification of 2A10BC. The fire extinguisher observed in the vehicle is a 1A10BC. During the inspection, Child Care Regulation provided technical assistance, Fire extinguishers with a minimum rating of 2A:10BC must be properly installed, serviced and maintained with current inspection tags at all times. Documentation of an accident or incident was insufficient in that it did not include the [first aid documented on the actions taken section of the accident report on 5/20/2025]. CCF Handbook, Section 6.4, C Health Requirements [SR] Comments: 39-5: During the inspection, Licensing Specialist observed that that an accident report was created on 5/20/2025 for child 1 getting bit by another child. Licensing Specialist observed that there was no first aid documented on the actions taken section of the accident report on 5/20/2025. Teacher 5 stated that the director brought teacher 5 to the office and showed her how to write an accident report and the procedures for when a child gets bit. The director told teacher 5 that if a child gets bit, the bite must be cleaned, and ice must be applied. Then the teacher must notify the office and then the office will call the parents. Then the teacher writes the accident report. This is being marked corrected at the time of the inspection. During the inspection, Licensing Specialist provided technical assistance, Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian.
