CCF Handbook, Section 3.5, F (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, Licensing Specialist observed on the older playground the gate had a gap greater than 3 1/2 inches. The gap was measuring at 6 inches. The director was given until August 15, 2025, to come into compliance with this standard. During the inspection, Licensing Specialist provided technical assistance, CCF Handbook Section 3.5 F- Outdoor Play Area. F. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or buildup to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard. CCF Handbook, Section 3.1, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: During the inspection, Licensing Specialist observed in the girls bathroom in the first stall there is a hole in the wall, in the lobby the door bell box is off the wall and hanging down, the exterior wall around the door of the 3 year old room is peeling, the exterior door frame of the 3 year old room is rusting and chipping, the laminate flooring is peeling up in the lobby and in front of the 1 year old room, the carpet by the cubbies in the 3 year old room is shredding, the ceiling tiles in the back room, and 4 year old room has brown and black circles and and bulging, the shelf in the 4 year old the shelf is peeling exposing the particle board and the table in the 4 year old the top of the table part of the Formica is missing and exposing the particle board, the shoe rack outside of the infant room is broken and the paint is peeling in the rooms.
