CCF Handbook, Section 2.4 (Section 2.1 - Health and Safety, Page 3) General Requirements [SR] Comments: 4-01: Licensing Specialist reviewed the camera footage from 1/20/2026 from 3:00 pm to 4:00 pm in the Two Year old Classroom. Licensing Specialist observed a few children playing with a cabinet at the 24:41 minute mark. The children were climbing inside the cabinet and closing the door. At one point teacher 1 attempted to count the children and walked over to where the children were playing with the cabinet. The teacher saw a child in the cabinet and got the child out. Another child, child 1, went into the cabinet after teacher 1 walked away at the 30:06 minute mark. The class and teacher 1 left the classroom at the 31:06 minute mark, leaving the one child alone in the cabinet. The child comes out of the cabinet, looks around the room, looks out the window and sits down against the wall. Another staff member, teacher 2, walked into the classroom at the 32:14 minute mark and saw the child alone. The staff member left the classroom with the child. Licensing Specialist reviewed the camera footage from 1/20/2026 from 3:00 pm to 4:00 pm on the Toddler Playground. Licensing Specialist observed that the two and three year old class came outside at the 31:02 minute mark with teacher 1. The video was not clear in the top half of the video footage due to the sunlight limiting the view. At the 38:19 minute mark, Licensing Specialist observed child 2 run out of the camera view behind the brown climber and into the section that was covered in the sunlight, obstructing the view. At the 39:08 minute mark, child 2 ran out from behind the brown climber and stood behind the green climber post (still in the sunlight with limited visibility). Child 2 appears to make a gesture of putting something over his head at the 39:17 minute mark. The child runs around and periodically touches his neck. Licensing Specialist saw that child 2 had the ziptie around his neck at the 40:40 minute mark. Child 2 goes over the teacher 1 at the 42:20 minute mark and teacher 1 acknowledges the teacher at the 42:32 minute mark. Teacher 1 is seen assessing the childs neck and Teacher 1 brings child 2 to the toddler rooms door at the 42:26 minute mark. Teacher 1 is standing at the Toddler door (the door is open). The director is seen at the 44:19 minute mark, cutting the ziptie off child 2s neck. Child 2 is brought inside with the director. The child might have had the ziptie around his neck for approximately 5 minutes (39:17 minute mark to 44:19 minute mark). Both incidents have been addressed and their parents were contacted immediately per the interview with the director and incident reports were created for the two incidents.
