Licensing Specialist observed 7 out of 28 children's health examination records that were expired or not present in the file during inspection. Licensing Specialist gave technical assistance from the Child Care Facility Handbook: 7.2 Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. C. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program. The facility's outdoor play area contained litter, nails, glass or other hazards that posed a threat to the health, safety or well-being of the children. CCF Handbook, Section 3.5, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: Licensing Specialist observed several bolts sticking out from the playground equipment that serve as holding the playground equipment structure together during inspection. LS observed several pieces of the same playground structure that were not connected to the playground structure at all. LS observed two ant hills present during inspection. LS gave technical assistance from the Child Care Facility Handbook: 3.5 Outdoor Play Area A. The outdoor play area must be clean and free from litter, nails, glass and other hazards Corrective Action: Provider is to submit photographic evidence to Licensing Specialist via e-mail of the playground structure being securely connected with all parts, including the bolts and pieces of the playground structure.
