The facility did not have documentation to show completion of a Department approved five-hour early literacy and language development course for child care personnel within 12 months of date of employment in child care industry and/or the early literacy course documentation was not uploaded in the Florida Pathways/Registry. CCF Handbook, Section 4.2.2 and 4.5, C Comments: The LS reviewed the training transcript for a staff member who has not completed Department approved Early Literacy Courses. 4.2.2 Early Literacy Training Pursuant to Section 402.305(2)(e)5., F.S., all child care personnel must complete a single course of training in early literacy and language development of children ages birth through five years that is a minimum of five clock hours or .5 CEUs. Early literacy training must be completed within 12 months of date of employment in the child care industry. Proof of completion may be documented on a certificate of course completion, classroom transcript, or diploma. Child care personnel must complete one of the following: A. One of the Departments online literacy courses available on the Departments website. B. One of the Departments approved literacy courses. A list of these courses can be obtained from the Departments website. (No additional courses will be approved by the Department.) C. One college level early literacy course (for credit or non-credit) if taken within the last five years. 4.5 Documentation of Training The Departments training transcript is the only acceptable verification of successful completion of the Departments training, early literacy courses and school-age training documentation. C. Early literacy courses and school-age training documentation must be uploaded into the Florida Pathways (Registry), which may be accessed from the Departments website at www.myflfamilies.com/childcare. Child/ren were placed in a crib, playpen, play yard or other sleeping and napping bedding with items that could pose a strangulation or suffocation risk. CCF Handbook, Section 3.6.4, A Physical Environment [SR] Comments: At the time of inspection, in the infants' room a LS observed blankets in the infant cribs. 3.6.4 Safe Sleep A. All personnel that care for infants must follow safe sleep practices as recommended by the American Academy of Pediatrics (AAP) as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Care and Education, which is incorporated by reference in 65C-22.001(7)(v), F.A.C. Cribs or playpens/play yards used for infants must have tight fitted sheets and no excess bedding, which includes but is not limited to: bumper pads, hanging mobiles, quilts, comforters, receiving blankets, pillows, stuffed animals and cushions During the inspection, child care personnel in the infants room removed the blankets from the cribs. 04. Supervision CCF Handbook, Section 2.4 (04-15)

Children did not receive supervision as required by their age or required needs while toileting or bathing. CCF Handbook, Section 2.4.1, H General Requirements [SR] Comments: At the time of inspection, the LS walked from the Multipurpose room and came around a corner in the hall to find a child S.F. unaccompanied by any staff. The LS observed the child and as they approached a class in the hall (toileting) the LS asked if the child belonged in the class. The staff member then asked the child if he needed to use the restroom then asked him to sit with the rest of the class. The LS returned to the multipurpose room and then headed back down the hall, where a student was walking away from the class that were being toileted. The LS observed the child K.M. enter a classroom, where the child laid on her nap cot. In the classroom there was no staff member in the room with the child for two to three minutes, until the class returned from the restroom. 2.4 Supervision 2.4.1 General Supervision Requirements H. Children must receive supervision and care in accordance with their age and required needs and be accounted for at all times while bathing or toileting. The child K.M. was adequately supervised when the class returned from the bathroom.
