   Child care personnel, including volunteers who work 10 hours or more per month did not complete the required in-service training during the states fiscal year beginning July 1 and ending June 30. CCF Handbook, Section 4.2.7 Training [SR] Comments: Licensing Specialist observed one staff member with employment date- 2/13/2024, child care industry start date- 10/16/2015 that completed the 40 hour introductory child care training courses on 10/29/2005. As of inspection date, the staff member did not complete the 10 hours of required in-service training during July 1, 2024 to June 30, 2025. Licensing Specialist gave technical assistance from the Child Care Facility Handbook: 4.2.7 Annual In-Service Training Upon successful completion of the 40-hour introductory training requirements, child care personnel must complete a minimum of 10 clock-hours or one CEU of in-service training annually during the states fiscal year beginning July 1 and ending June 30. Corrective Action: Provider is to submit to Licensing Specialist via e-mail the one staff member's 10 hours of in-service, including the in-service log form & the certificates/supporting documentation of the training taken. ***Please note that the trainings taken to meet this requirement will not be able to be utilized towards the current fiscal year, July 1, 2025 to June 30, 2026.    Employment history check did not include all required information including applicants, job title, description of regular duties, confirmation of employment dates, and level of job performance. CCF Handbook, Section 5.1, D Record Keeping [SR] Comments: Licensing Specialist was not able to observe 5 staff members complete 5 year history with reference checks conducted during inspection, due to the information not being on file. Licensing Specialist gave technical assistance from the Child Care Facility Handbook: 5.1 Initial Screening A screening must be conducted as a condition of employment. The employer/owner/operator must review each employment application to assess the relevancy of any issue uncovered by the complete background screening, including any arrest, pending criminal charge, or conviction, and must use this information in employment decisions in accordance with state laws. D. The employer/owner/operator must conduct employment history checks, including documented attempts to contact each employer that employed the individual within the preceding five years, and documentation of the findings. Documentation must include the applicants job title and description of his/her regular duties, confirmation of employment dates, and level of job performance. The employer/owner/operator must make at least three attempts to obtain employment history information. Failed attempts to obtain employment history must be documented in the personnel file and include date, time, and the reason the information was not obtained. Corrective Action: Provider to obtain a complete 5 year employment history with reference checks conducted for the 5 staff members.
