While conducting a ratio head count for staff to children, it was observed that the summer camp and two-year-old teachers did not have an accurate count reflecting on the attendance roster to what they had in front of them. Some children were moved around to other classrooms and staff failed to ensure arrival and departure times were complete and accurate at the time of arrival or departure. CCF Handbook, Section 2.5.1 General Requirements [SR] Comments: Licensed Specialist reviewed the physician certification for driver, C.G., and saw it expired in April 2026. The driver stated she drove the vehicle the whole month of May. When Licensing Specialist asked to review the transportation logs for the month of May, the provider gave licensing specialist logs missing the drivers signature. When asked about the signatures, the provider took the paperwork to look at and stated they were the wrong documents, licensing specialist asked for them back and she crumbled them in front of the specialist and threw them away. She was unable to provide the documents at the time of inspection. She emailed them to the licensing specialist on 6/14/26 at 4:08P.M., showing herself as the driver. She was asked if C.G., drove at all during the month of May, but she never answered. This was resolved when the driver, C.G., had an altercation with the provider on 6/17/26 and has not returned. 2.5.1 Driver Requirements The driver of any vehicle used by a child care program to provide transportation must have the following: A. A valid Florida drivers license including the proper endorsement; B. General Requirements [SR] Comments: Licensing Specialist observed the infant classroom to have 12 children: mixed ages with 1 childcare personnel. Childcare personnel stated there was 1 other teacher but was in the restroom at the time. The second teacher came in the room and tried to assist the teacher in the room. Licensing Specialist observed the following ratio: 1 infant, 1-1 yr. Old, 2-2 yr. olds, 6-3 yr. olds and 2-4 yr. olds. Licensing Specialist also noticed an 8 yr. old boy asleep alone in a separate classroom unattended. (The child was asleep in the VPK classroom when you first walk in the front door). So that would be a total of 13 children and 2 childcare personnel. This was resolved when another teacher and the Director arrived. They separated the children into other classrooms. The following ratios were observed before the Licensing Specialist completed the inspection: Class 1: 15 VPK students and 3 childcare personnel. Also, there were 2-3 yr. old students in the same room in the far side of the room practicing dance with an instructor. Class 2: 4 Infants and 1 childcare personnel. Class 3: 6-2 yr. olds and 1 childcare personnel. Class 4: 10-3 yr. olds and 1 childcare personnel. 1 additional staff was available at the time of inspection. Ratio standards were in compliance by the time the Licensing Specialist was finishing with the inspection. 2.3 Ratios The staff-to-child ratio, as established in Section 402.305(4), F.S., is based on primary responsibility for the direct supervision of children and applies at all times while children are in care. 2.3.1 Mixed Age Groups A.
