   The facilitys fencing walls or gate area had gaps that could allow children to exit the outdoor play area. CCF Handbook, Section 3.5, F (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: Licensing Specialist observed the front gate of the fence of the center to have more than a 3.5 inch gap. The latch was not placed on the door and it was easily opened. The gap was covered by a small chair in front. Licensing specialist let provider know that it needs to be a secure gate/fence entrance since the gap or unlatched fence could allow children to exit outdoor play area. Technical Assistance was provided F. The outdoor play area must have adequate fencing or walls a minimum of 4 feet in height. Fencing, including gates, must be continuous and must not have gaps or opening larger than 3 inches that would allow children to exit the outdoor play area. The base of the fence must remain at ground level and be free from erosion or buildup to prevent inside and outside access by children or animals. These areas must have at least two exits, with at least one being remote from the buildings. If an outdoor play area was approved for usage by the Department prior to October 25, 2017, no new exits are required to be added to meet this standard.    The play equipment was not maintained in a safe or sanitary condition for the children to use in that [Licensing Specialist and other agent observed two metal bus frames on the outdoor playground. One bus had a tire that had a large hole in it with a large screw/nail inside. Both equipment buses were rusty.]. CCF Handbook, Section 3.12, B and F (Section 2.1 Health and Safety, Page 3) Sanitation and Equipment [SR] Comments: Technical Assistance was provided. 3.12 Outdoor Equipment B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. C. All equipment, fences, and objects on the facilitys premises shall be free from sharp, broken and jagged edges, and properly placed to prevent overcrowding or safety hazards in any one area. F. All equipment used in the outdoor play area must be constructed and maintained according to manufacturers recommendations and allow for water drainage.
