   A ratio of [2] child care personnel for [7- 1 year old children.] children is required. A ratio of [1] child care personnel for [7- 1 year old children.] children was observed. s.402.305(4), F.S. Comments: During onsite visit the specialist observed 7- 1 year old children present with 1 staff member. The item was corrected at the time of inspection when one child was sent over to the infant room that had 3 infants at the time bringing the room to 4 children with 1 staff member and the 1 year old class was brought back into compliance with 1 staff with 6 1- year old's. The staff member stated that the child that put them out of ratio is not normally in attendance at the center and with the child being in attendance put them out of ration. Specialist recommended to either create a 1 year old class or move the child over to another classroom as soon as they come in and do not accept the child in the classroom. Violation was informed to management to ways to prevent and was informed by management that the child is usually not in attendance and will speak with the parent. 2.3 Ratios. In groups of mixed age ranges, where children one year of age but under two years of age are included, one child care personnel must be responsible for no more than six children of any age group, at all times.    The Florida Certificate of immunization was not acceptable in that: [ The form was not current (expired)]. CCF Handbook, Section 7.1 Comments: Refer to the supplemental for the child who had an expired form on file during onsite inspection. Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Child Care Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. A. Immunizations received out-of-state are acceptable; however, immunizations must be documented on the Florida Certification of Immunization form and must be signed by a physician practicing in the State of Florida.    44. Personnel Records CCF Handbook, Section 7.4 (44-01)

   Personnel records or copies of records were not being maintained at the facility and available for review by the licensing authority. CCF Handbook, Section 7.4 Comments: The facility is out of compliance with this standard as a potential staff member who was at the center for a "onsite work day", was allowed to be alone with a child to change his diaper and supervise enrolled children on the playground without having a complete file folder onsite for review. Even though the potential staff member is no longer associated with the center, the specialist recommends that the facility derive a safety plan that will describe steps that they will take to prevent future issues of this nature occurring again. A copy of the safety plan is to be provided to the specialist for corrective action. 7.4 Personnel Records Records must be maintained and kept current on all child care personnel, as defined by Section 402.302(3), F.S. These records shall be on-site, available for review by the licensing authority and must include: A. A complete employment application with the required statement pursuant to Section 402.3055(1)(b), F.S. B. Documentation of position and date of employment. C. CF-FSP Form 5337, Child Abuse & Neglect Reporting Requirements, which is incorporated by reference in 65C-22.001(7)(l), F.A.C., must be signed on or before hire date and annually thereafter by all child care personnel.
