Child care personnel failed to document an accident or incident on the day that it occurred. CCF Handbook, Section 6.4, A Health Requirements [SR] Comments: Licensing Specialist observed the incident/accident report for child # 1. LS observed that there was no first aid written on the incident report that was given to child #1. LS observed that there was no written documentation of the parent being notified of the incident. LS observed that the parent did not sign the incident report and that facility wrote that the parent refused to sign the incident report. Licensing Specialist did not observe an incident/accident report for the child that scratched child #1 due to an incident/accident report not being on file. Licensing Specialist gave technical assistance from the Child are Facility Handbook: 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. Corrective Action: Provider is to submit to Licensing Specialist a written statement that the facility & staff members understand and will maintain Section 6.4 from the Child Care Facility Handbook. 40.
