All adults and children failed to evacuate the facility when the approved fire alarm system was activated. CCF Handbook, Section 3.8.4, B (Section 2.1 - Health and Safety, Page 3) Physical Environment [SR] Comments: Specialist observed a fire drill on site at time of inspection. During the fire drill, specialist found a child from classroom B left inside from the evacuation. Specialist brought the child outside to the rest of the classroom and spoke with the provider. The facilitys record for a child receiving medication was incomplete in that it did not include [amount given]. CCF Handbook, Section 6.5, F Health Requirements [SR] Comments: Specialist observed two medications without medical authorization forms. In Classroom A for A.R and Classroom B for K.C. The medication forms located during inspection missed a description of dosage. The medication form F. The facility must maintain a record for each child receiving any medications that documents the full name of the child, the name of the medication, the date and time the medication was given, the amount and dosage, and the name and signature of the person who gave the medication. This record must be initialed or signed by the program personnel who gave the medication. The record must be maintained for a minimum of 12 months after the last day the child received the dosage.
