The vehicle alarm installed was not in working condition. By January 1, 2022 all vehicles used by childcare facilities to transport children must be equipped with a reliable alarm system approved by the Department which prompts the driver to inspect the vehicle for children before exiting the vehicle. 1. Approved alarm systems must meet the following criteria: a. The alarm system must be armed or activated automatically when the vehicles ignition is turned on. b. The alarm system must be designed and installed so that the vehicle horn, siren or other type of audio alarm will sound if the driver/staff member does not walk to the rear or, in the case of a passenger van, the side entry point of the vehicle, to manually shut off or deactivate the alarm. c. The time delay from the time the ignition is turned off after activation of the alarm system until the alarm sounds shall be no longer than one minute. d. The alarm must be audible from the distance of 500 feet from the vehicle. e. The alarm system must be installed so that the driver must walk to the back of the vehicle to reach the deactivation mechanism. Deactivation mechanisms installed in locations that do not require the driver to walk to the back of the vehicle and view all seating areas will not be acceptable. 2. List of approved alarm manufacturers may be found on the Departments website at www.myflfamilies.com/childcare . 3. Alarms must be installed and maintained according to the manufacturers recommendations. 4. Alarm systems installed according to the manufacturers instructions prior to October 1, 2021, will be deemed to satisfy this requirement if they meet the criteria set forth in 2.5.4.F.1. above. Under this allowance, the existing alarm system must always be properly maintained in working order. Should at any time the alarm system require replacement, the new system must be selected from the Departments approved list referenced in 2.5.4 F.2. above. 08. A ratio of [2] child care personnel for [(18) two-year-old & (1) three-year-old] children is required. A ratio of [1] child care personnel for [18 two-year-old & 1 three-year-old] children was observed. s.402.305(4), F.S. General Requirements [SR] Comments: Based on the staff reports and attendance records it was discovered that on 9/30/2025 only one staff was supervising a group of (18) two- year-olds and (1) three-year-old. One staff was supervising 19 children total. Supervision of children in the [twos ] group was inadequate in that [the ratio was high preventing adequate supervision causing a child to be harmed]. The facility did not maintain the sink, toilet and/or bath facilities used by the children in good operating condition, clean and sanitized or disinfected, posing a threat to the health, safety or well-being of the children. CCF Handbook, Section 3.7, I Physical Environment [SR] Comments: Licensing counselor observed the toilets in one class (Pre-K Room 2) to be soiled around the seats and at the ground level around the toilet. Licensing counselor provided technical assistance regarding maintaining a clean toilet and wiping the seat after children are done using it. From the Child Care Facility Handbook Section 3.7:Each toilet and sink must be maintained in good operating condition, cleaned and sanitized or disinfected as needed, but at least once per day. Sinks utilized for teeth brushing activities/procedures by the children must be sanitized prior to the activity occurring. TA: Facilities must have a written routine schedule for cleaning, sanitizing and disinfecting equipment, materials, furnishings and play areas. This schedule must include items to be cleaned, sanitized or disinfected on a daily or weekly basis. CCF Handbook, Section 3.9.3, E Food and Nutrition [SR] Comments: The staff confirmed that she allowed the child to consume food brought from her home. The program's policy regarding "employee foods" states that "consumption of food by employees, other than posted menu items, is prohibited in unauthorized areas such as occupied classrooms" The Director sent a statement via e-mail on 7/11 to confirm the staff was coached and reminded of the Program's Policy, as well as signed a Corrective Action (attached). Per the Child Care Facility Handbook Section 3.9.3 Food Safety: Parents and legal guardians must be advised in advance of each food related activity, such as special occasions and learning activities that include food consumption. Written parental permission may be obtained in the form of a general or specific permission slip.
