A prescription or non-prescription medication, specifically [Nystatin and RX Hydrocortisone], was dispensed without written authorization from the custodial parent or legal guardian. CCF Handbook, Section 6.5, A and K (Section 2.1 Health and Safety, Page 3) Health Requirements [SR] Comments: Specialist observed two RX medications, Nystatin for A.S and Hydrocortisone for J.J. Both medications were prescription and did not have medication authorization forms. Per the CCF Handbook A. The child care program must have written authorization from the custodial parent or legal guardian to give prescription and non- prescription medications. This authorization must be dated and signed by the custodial parent or legal guardian and contain the childs name; the name of the medication to be given; and date, time and amount of the correct dosage to be given. Prescription and non- prescription medications that are used on an as needed basis require the parent/ legal guardian to provide additional documentation on the authorization form to describe symptoms that would require the medication to be given. The child care provider must never administer a medication that is prescribed for one child to another child. Technical assistance was provided regarding medication authorization forms and maintaining records. At time of inspection, medications were either thrown away or returned to the parent. Cleaning (other than general clean-up activities) of a room took place while children were present in the room. CCF Handbook, Section 3.1, G (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Comments: Specialist observed in classroom C personnel utilizing spray-bleach to clean toys on a short table as the children were laid down for nap in the same classroom. Per the CCF Handbook, 3.1 General Health and Safety Requirements G. Cleaning must not take place while rooms are occupied by children except for general clean-up activities that are part of the daily routine. General cleaning refers to cleaning necessary to maintain a sanitary environment but that does not pose a hazard to children, such as wiping the table after lunch, soaking toys in a tub on the countertop, or sweeping. This does not include cleaning with hazardous materials or any cleaning which poses a risk of slipping or falling. Specialist interrupted the personnel as they were cleaning and explained that they should be utilizing non-hazardous cleaning supplies away from the children as they rest. A different teacher came over and continued instruction based on the facility's procedures on cleaning. Technical assistance was given regarding cleaning procedures and training staff on the posted cleaning schedule. 15.
