Compliance (Annual) The following deficiencies were observed at the time of the compliance inspection conducted on March 20, 2026, and are subject to changes pending programmatic review. 4 of the 4 Fingerprint clearance cards reviewed were valid via a DPS website search. Please submit the Plan of Corrections on the Licensing portal within 10 days of receiving this Statement of Deficiencies. The Emergency Disaster Contact Form was completed during the inspection. BCCL staff emailed the Empower Self-Evaluation Assessment link to the Provider. The following items were discussed, but not limited to: **Criminal History Affidavits had the 2nd page attached to the signed document **TB screening form is completed for all staff **Child Assistant Educators are supervised by a qualified staff Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
