Compliance (Annual) The following deficiency was observed at the time of the Compliance Inspection conducted on 8/11/2025 and are subject to changes pending programmatic review. A paper copy of the Notice of Inspection Rights was given to the Director at the time of the inspection. Please submit the Plan of Corrections via the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Empower Self-Evaluation invitation was emailed to the facility. The fingerprint clearance cards for 2 of 2 staff members were verified to be valid through the DPS website at the time of the inspection. During the exit interview, the following items were discussed, but are not limited to: *Maintain 6” of wood chips in the fall zones of the outdoor activity areas *In the 1s classroom, the gate on the portable play yard will be removed or an application to update the square footage of the classroom will be submitted *Brooms will be inaccessible to enrolled children *Staff must have a valid fingerprint clearance card within 30 days of their 18th birthday Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
