The following deficiencies were observed at the time of the Complaint #00168559 investigation conducted on 5/5/26 and ending on 5/6/26 and are subject to changes pending programmatic review. A full inspection was not conducted at this time. ***The complaint was self-reported by the facility on 5/3/2026. A Plan of Corrections will not be accepted at this time. The following ratios were observed: *Fours/Fives: 3:7; 3:9; 3:20; 3:14; 5:7; 5:22; 3:14; 3:16 Two staff members and the Facility Director were interviewed during this investigation. The following documentation was reviewed at the time of the inspection: Written Statements, IEP Upon completion of the Complaint investigation, it was determined from facility documentation and staff interview that 1 of 1 allegation was substantiated.
