Complaint The following deficiencies were observed at the time of the Complaint #00121685 Inspection conducted on 3/07/2025 and are subject to changes pending programmatic review. A full inspection was not conducted at this time. A paper copy of the Notice Of Inspection Rights and the Small Business Rights was given to the Director. A Plan of Correction will not be accepted at this time. The Compliance Officer was unable to make contact with the complainant. Ratios observed were: Infants: 2:6 1’s: 2:11 2's: 2:7 2’s/3's: 2:10 3’s: 2:16 4’s/5’s: 2:17 There were 4 staff interviewed during this investigation Documentation observed: Emergency, Information, and Immunization Record cards, classroom rosters, 3 staff files, and a video clip. The fingerprint clearance cards for 3 of the 3 staff members were verified to be valid through the DPS website at the time of the inspection. Upon completion of the complaint investigation, it was determined from observation, interview, and documentation, that 1 of 2 allegations was substantiated and 1 of 2 allegations was unable to be substantiated due to lack of sufficient evidence.
