Compliance (Annual) The following deficiencies were observed at the Compliance Inspection conducted on 10/15/2025, and are subject to changes pending programmatic review. Please submit the Plan of Corrections via the LMS portal within 10 days of the receipt of the Statement of Deficiencies. A copy of the Notice of Inspection Rights was provided at the time of the inspection. 5 of 5 fingerprint clearance cards were verified to be valid through the DPS website during the time of the inspection. The following items were discussed, but not limited to: 1) New rules effective 8/3/2025, including TB attestation, orientation, 24 hours of annual training hours, email address on emergency cards, and the Expulsion and Suspension policy added to the Statement of Services. Please submit the following documentation: 1) Staff #3’s foreign degree equivalency verification; and 2) Revised parent handbook. Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
