The following deficiencies were observed at the time of the Compliance Inspection conducted on 06/18/2025. A copy of the Notice of Inspection Rights was given to the Provider. Please submit the Plan of Correction in the LMS portal within 10 days of receip
Compliance (Annual) The following deficiencies were observed at the time of the Compliance Inspection conducted on 06/18/2025. A copy of the Notice of Inspection Rights was given to the Provider. Please submit the Plan of Correction in the LMS portal within 10 days of receipt of the Statement of Deficiencies. The Empower Survey link and Emergency Disaster Contact Form was emailed to the Provider. The fingerprint clearance cards for 2 of 2 staff members and 2 of 2 residents were verified to be valid through the DPS website at the time of the inspection. A monitoring inspection will be scheduled. During the exit interview, the following items were discussed but are not limited to: *Diaper changing surface will be sanitizable *Maintain a written record of the notification of illness in the child's file *Unused outlets will be covered with a safety plug *Attendance records will include a time and signature for each admission and release of the child *First Aid kit will be maintained with gauze rolls *The current childcare license will be posted *Documentation of annual training hours will be maintained *Maintaining Supervision if using a tent Result: Complete Source: AZ Care Check (ADHS Bureau of Child Care Licensing).
