The director self reported on February 13, 2026, child care worker(s) failed to supervise a three-and-a-half-year-old child when the child was left outside for approximately one minute in the fenced in play space. Child Tracking Procedure (251.055(1)(f))

On February 13, 2026, child care worker(s) failed to accurately implement the program's child tracking procedure. A child failed to be signed into the bright wheel app indicating the accurate number of children present. When the workers completed a head count the number of children indicated on the app did not accurately reflect the number of children present. This resulted in a three-and-a-half-year-old child being left outside for approximately two minutes.
