On 1/30/25, a serious injury of a child occurred on climbing equipment; the mulch was measured at approximately one inch. The center did not follow their playground safety procedures.

Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. There was a container of disinfectant wipes in the bathroom that two-year-old children use. The wipes were unlocked and had a warning label that stated ?Keep Out of Reach of Children.?
