Areas and equipment of the center, inside and outside, shall be maintained in a safe condition. On the toddler playground, several rungs of the vinyl fence gate were broken and cracked, creating a pinching and snagging hazard. There was also peeling paint on the outside metal baseboard of a building window and deteriorating wood and peeling paint on the outside of the shed located on the playground.

Documentation shall be maintained of shelter-in-place and lockdown drills that include all the required elements. The drill log for the center did not include the following elements for the documented shelter-in-place and lockdown drills: identity of the person conducting the drill, the method used for notification of the drill, the number of staff and number of children participating, any special conditions simulated, the time it took to complete the drill and problems encountered, if any.
