The center's written procedures for safe sleeping practices and sudden infant death syndrome awareness states infants should be placed on a firm sleep surface (such as a crib mattress). Infant #1 (age 7 months) fell asleep in a swing and staff #1 left the infant sleeping in the swing for over an hour. The swing also contained a "Warning" on the tag that stated "this product is not safe for sleep."

Staff shall wash their hands with soap and running water before and after a diaper change and children's hands shall be washed with soap and running water after diapering. 1. In a 2 year old classroom, staff #4 did not wash hands before changing two children. The licensing inspector reminded the staff of the requirements of the standard. Following the reminder, Staff #4 proceeded to diaper another child without washing hands. 2. In the infant classroom, after diapering, staff #12 did not wash their hands nor the hands of infant #4.

REPEAT VIOLATION Staff shall ensure the immediate safety of a child during diapering and the disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. 1. During the inspection, child #6 (age 2) was observed lying alone on the diapering table in the Jack & Jill bathroom while Staff #4 remained in the 2-year-old classroom. 2. During two diaper changes in the Prep B classroom, Staff #4 stepped away from the diapering table to dispose of diapers, leaving each child unattended on the table. 3. During the inspection, Staff #4 lifted the trash can lid with their hand while holding a soiled diaper and disposed of it in this manner after three diaper changes.

Staff #6 only sprayed the diaper changing surface with a sanitizing solution, but did not allow it to dry for a period of 60 seconds or air dry, as the directions state.

Both staff only sprayed the sanitizer on the surface and immediately wiped it away, but the directions on the sanitizer states to leave the sanitizer for 60 seconds before wiping away or air drying.
