500,B,5 - Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Staff #4 changed a diaper and disposed of it in an plastic grocery style bag that was on top of a foot-operated trash can and then taken outside to a dumpster where the staff used their hands to carry the grocery bag and then to lift the dumpster lid to dispose of it.

The medication authorization shall be available to staff during the entire time it is effective. Prescription medications were observed on-site for child #2, child #3, and child #7. There were no parent authorizations and no physician authorizations to administer the medications.

Tables shall be sanitized before and after each use for feeding. In the three and four year old classroom, child #2 was observed eating a snack. The table was not sanitized after the child finished.

The record for Child #1, enrolled for four years, was missing the following required components: an address for one emergency contact, phone number of child?s physician and annual update. The record for Child #4, enrolled for six weeks, was missing the following required components: one emergency contact. The record for Child #5, enrolled for one year, was missing the following required components: one emergency contact, employment information for the parents, and annual update.

REPEAT VIOLATION The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. There was chipped paint and visible dirt on the door frames withing in reach of the children. Parts of the large multi-sectional playmat in the infant room and a foam block in the toddler room were peeling.

Staff shall wash hands with soap and running water before feeding and children?s hands shall be washed with soap and running water before eating. Staff #5 and staff #6 did not wash hands before feeding two infants and the two infant?s hands were not washed before eating.
