For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. Attendance had not been completed in the PreK classroom since April 5th, 2026.

The door to the kitchen was unlocked and therefore accessible to children in care during this inspection. There were several bottles of cleaning chemicals in the kitchen.

The center did not follow their policy on diapering. On 2/24/26 and 3/18/26 a child was not changed prior to leaving the center as per their policy.

When staff are supervising children, they shall always ensure their care, protection, and guidance. During this inspection child #5, three months old was sleeping in baby swing swaddled in a sherpa lined blanket that came up around their face.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include future action to prevent recurrence. The injury reports dated 7/14/25 for child #7 and 8/14/25 for child #8 did not contain future action to prevent recurrence.

Prepared infant formula shall be refrigerated, dated and labeled with the child's name. The bottle labeled with the name of child #6 was not dated.

A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #2, who has been enrolled for approximately two and a half years, has a diagnosed food allergy and did not have a written care plan in their file.

This list shall be dated and kept confidential in each room or area where children are present.. The children's allergy list was not dated and confidential.

Electrical outlets shall have protective covers that are of a size that cannot be swallowed by children. An electrical outlet in the yellow room did not have a protective cover on it.
