The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan required in 8VAC20-780-60 A 8. The center allergy list did not include a food sensitivity/dietary restriction for child 1 and child 2. The enrollment records for both children indicate a food sensitivity and/or dietary restriction.

Repeat Violation Staff records shall contain all required elements. The staff record for staff 1, staff 2 and staff 3 lacked written documentation to demonstrate completion of orientation training.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include all required elements. *4 injury reports lacked documentation of the time the parent was notified of the injury; *1 injury report lacked a parent or second staff signature; *3 injury reports lacked documentation of any future action to prevent recurrence of the injury.

Each child shall have a physical examination by or under the direction of a physician before the child's attendance or within the 30 days after the first day of attendance. Child 3 lacked documentation of a physical exam. Child 3 has been enrolled for over two years.

Documentation of the required tuberculosis screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 days of the date of employment. The TB screening for staff 3 was completed after the first date of employment.

Staff records shall contain all required elements. The employee file for staff 1 and staff 2 lack written documentation to demonstrate that they possess the education and training required by their job description. Both staff have a job title of program leader and were observed functioning as a program leader during the inspection.

The center shall ensure that during transportation of children the following information is in transportation vehicles: *A list of the names of the children being transported. A list of the names of the children being transported was not on the facility van when the vehicle arrived from local public schools at approximately 3:05 pm.

Based on record review and interview, the center failed to ensure that the current written list of all children's allergies, sensitivities, and dietary restrictions shall be current and include all required information. Evidence: 1. The written list of children's allergies, sensitivities, and dietary restrictions did not include written information for child 1 with a food sensitivity listed on her enrollment form. 2. The program director confirmed that child 1's food sensitivity was not included on the allergy list for the facility.

Based on observation and interview, the center failed to ensure that a 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center shall be posted in a visible place at each telephone. Evidence: 1. The phone number for 911, poison control or local dial for local fire, police and emergency medical services were not posted by the center telephone. 2. The program director confirmed that these emergency phone numbers were not posted.

Based on observation and interview, the center failed to ensure that shelter-in-place procedures or maps shall be posted in a location conspicuous to staff and children on each floor of each building. Evidence: 1. The posted evacuation map did not indicate any shelter-in-place procedures. 2. The program director confirmed that procedures for shelter-in-place were not included on the posted evacuation map for the facility.
