The sponsor shall maintain public liability insurance for bodily injury for each center site with a minimum limit of at least $500,000 each occurrence and with a minimum limit of $500,000 aggregate. There was no documentation of current liability insurance available to be reviewed during the inspection.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information which includes work phone number and place of employment of each parent who has custody and the name, address, and phone number of two emergency contacts. Child 3's record was missing the work number for one parent. Child 4's record was missing the employment information for one parent and two emergency contacts. Child 6's record was missing one emergency contact.

Repeat Violation: For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. There were seven children present in the infant classroom but six children were signed in on the attendance sheet. There were nine children in the preschool classroom but ten children were signed in on the attendance sheet.

Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. The bathroom in the preschool classroom had a package of wipes on the floor. The changing pad had debris on it. The wooden doors to the fence outside the preschool classroom has gaps at the top and bottom creating an entrapment hazard. There is a slat missing from the same fence creating an entrapment hazard. The fence door leading to the larger playground does not latch properly as the doors are uneven. The top of the fences is curved upward with protruding nails. The garden box near the playground has protruding nail/bolt.

When staff are supervising children, they shall always ensure their care, protection, and guidance. The fence door leading to the infant playground was left open and unsecure, The fence being opened allowed the general public to have access to the entire outdoor play area.

The staff-to-child ration for children ages birth through 16 months is 1:4. There were seven children in the care of 1 teacher from 12:00 pm through 12:15. The youngest child in the room was 11 months.

Child 1 and child 2's records did not have documentation confirming that the required information was reviewed by the parent annually and up-to-date. Child 1's record was last reviewed on 4/2/25. At least every two years from the date of the first initial screening or testing, staff shall obtain and submit the results of a follow-up tuberculosis screening. Staff 1's tuberculosis screening was overdue by one year and two months.

There were 9 children observed in the Ducks classroom (three and four year old) but 7 children documented on the written attendance form.

On 5/1/25 at approximately 2:10pm, there were seven children observed present for the Ducks class (three-year-olds). The attendance record contained documentation of eight children present. The eighth child had left at approximately 12:20pm.
