Child A turned 4 years old, 6 months ago and the last vaccination record was completed over 3 years 6 months. 2. Child B?s (enrolled 3 years) record did not contain documentation of additional immunizations once between the child?s fourth birthday. Child B turned 4 years old, 11 months ago and the last vaccination record was completed over 4 years 8 months.

Centers shall keep a record of medication given to children. The center currently administers 3 different medications to Child A, once a day. The center has not kept a record of the medication administered to Child A in over 55 days.

When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. The authorizations for medications belonging to Child B and C expired over 2 months ago. The parent did not pick up the medications and the medications were not disposed of by the center.

When a child is placed in an infant seat or highchair, the protective belt shall be fastened securely. In the Infant A Classroom, an infant, under 12 months, was sitting at a feeding table and the protective belt was not fastened securely.

Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. In the Pre-K A classroom, soiled diaper were observed in a trashcan that was foot-operated but did not have a lid.
