The center is required to keep a current list of all known allergies, sensitivities, and dietary restrictions in each room or area where children are present. The list must be dated and kept confidential. The allergy lists in the infant/toddler room and in the Pre-K room were not dated and had previous children's information who no longer attended the center.

Staff shall complete a minimum of 16 hours of training annually, appropriate to the age of children in care. Staff #3 obtained no training hours in 2024. Staff #4 completed 12 of the 16 required hours in 2024.

When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. Medication for Child #2 was onsite, but the medication authorization had expired over 60 days ago. The medication had not been administered past the expiration of the authorization. The authorization had not been renewed and the medication was not returned or disposed of as required.

Staff who work directly with children shall, in addition to preservice and orientation training required in subsections A through D of this section, annually attend at least 16 hours of training and staff development activities, to include the department's health and safety update course. Staff #3 did not complete the annual health and safety training in 2024.
