The deck area used to access the playground had deteriorating splintered wood that could snag clothing or skin. Two deck boards were warped posing a tripping hazard.

The facility is required to ensure that the resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standards. On the playground, the fall zones around the climbing equipment were below 6 inches.

Hazardous substances must be in a locked place that prevents access by children. In the preschool room, a container Lysol sitting on a cabinet and was not locked and out of reach of children.

Playground equipment with moving parts or climbing apparatus must have appropriate fall zones that encompass sufficient area to include the child's trajectory in the event of a fall. The fall zone under the gymnastic rings did not have a resilient surface. The fall zones underneath and surrounding the equipment require at least six feet of resilient surfacing.
