When staff are supervising children, they shall always ensure their care, protection, and guidance. Child #1 (4month old) was sleeping in an infant seat at the time of inspectors arrival. When staff #3 moved child #1 to a crib they placed child #1 face down on their stomach. Staff #3 then picked child #1 up by their arm and same side leg to reposition child #1 from their stomach to their back.

The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence. The record shall include future action to recurrence. The injury report dated 04/10/2026 did not contain future action to prevent recurrence.

After the first license, annual approval from the health department shall be provided. The health inspection was overdue by 1 month, the last inspection was dated 04/16/2025.

Staff shall have current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care. Staff #3 did not have current certification in CPR, this was overdue by 2 months.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. There was exposed weed barrier intermingled with the playground mulch, presenting potential trip hazards. There was cracked/chipped drywall in some of the classrooms. The standup freezer in the Toddler room was unlocked.

Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children. Sanitizer and Lysol were in the unlocked cabinets above the toileting area in the3-4's room and the toddler room. Lysol was also found on the rolling cart in the 2-3's room.

In areas used by preschool age or younger children, electrical outlets shall have protective covers. There were uncovered outlets in the 2-3's room and Infant room that were within the reach of the children in care.

When staff are supervising children, they shall always ensure their care and protection. On 3/24/2025, Staff #1 mopped the floor in the toddler classroom prior to the children leaving the room to go to the front of the building for dismissal. Staff #2 and #3 reported that they were aware the floor had been recently mopped by staff #1 and wet floor signs were put out. Staff #1, 2, 7, and 8 stated the center's procedure is that the floor is not to be mopped until after the children leave the classroom for the day. Staff #2 and #3 walked 10 children ages 16 10 24 months, across the wet classroom floor towards the front of the building. Child #1 fell at approximately 4:00pm on the wet floor. Staff #2 helped child #1 off the floor and continued to walk through the hallway, holding child #1 up by the arm. Child #1 was observed limping and was picked up by staff #3. When parent picked up child #1 at approximately 4:08 pm she was informed that child fell due to the floor being wet. Parent took child to the ER because she was limping and did not want to put any weight on her leg. ER diagnosed her with a "toddler fracture".

For staff hired before July 1, 2024, providers must obtain the results of a central registry search for an employee, within 30 days of service. Caregiver #2 did not have central registry search results during the inspection and has been employed for over 90 days.
