When family day homes provide meals or snacks, the following shall apply: Family day homes shall follow the most recent, age appropriate nutritional requirements of a recognized authority such as USDA. The snack served to children contained one food component which was a fruit squeeze of applesauce. USDA requires two separate food components for snack. The lunch served to children did not include the required five food components. The lunch lacked fluid milk and a fruit or second vegetable serving. Each child received two or three pizza rolls which did not contain the required protein serving for the ages of children in care.

Repeat Violation Soiled disposable diapers and wipes shall be disposed of in a leak-proof or plastic-lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal. The diaper disposal system in use requires caregivers to use their hands to push the diaper down into the diaper disposal container.

Furnishings of the family day home shall be maintained in a safe condition. 2 of 4 cribs reviewed lacked any type of documentation to demonstrate that they meet CPSC mandatory federal standards for full-size and non-full size cribs. Both cribs were occupied at different times during the inspection.

In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities. Caregiver 2 has completed 13 of 16 hours of required annual training for 2025 according to written documentation provided.

A food allergy/sensitivity listed on the enrollment form for child 1 was not included on the food allergy list maintained by the provider.

The TB screening for caregiver 1 was obtained 60 days after hire and contact with children.

The inspector observed a plastic grocery bag, containing two soiled diapers, placed on the ground outside of the child care entrance.

The provider shall ensure that a caregiver does not exceed 16 points by using the following point system to determine if an additional caregiver is needed: *Children from 16 months through 23 months of age count as three points each; *Children from two years through four years of age count as two points each. Caregiver 1 was observed alone with 7 children whose ages counted for a total of 17 points.
