The following actions or threats thereof are forbidden: Physical punishment to include roughly handling as punishment. At approximately 9:45 am on 03/11/2026 Staff 1 was observed roughly handling child 1, age 3 years, when she used a quick aggressive motion to grab child 1 under both arms while moving her onto the carpet with sufficient force to cause her entire body to jerk and head to snap back. Staff 1 stated, ?Don?t you move again.? When child 1 crawled back onto the floor, staff 1 got up from the chair she was sitting in and again used a quick jerking motion to pull child 1 up from the floor by her arm. Child 1?s feet left the floor as she was pulled up by staff 1 and roughly placed onto the carpet.

Staff who work directly with children shall annually attend the department's health and safety update course. Staff 1 and staff 2 lack documentation of completion of the department's health and safety update course for 2024.

The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment. There is no documentation that staff 1 and staff 2 have completed The Virginia Department of Education sponsored orientation course.

Staff shall complete annually a minimum of 16 hours of training appropriate to the age of children in care. Staff 1 and staff 2 lacked documentation of 16 hours of annual training for 2024 to present. Both staff have been employed for more than 4 years.

Documentation of the required tuberculosis shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment. The TB screening for staff 1 was completed more than 45 days before her first day of employment.

Systemic Deficiency Areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin. 1. The following safety hazards were observed on the outdoor play area: a. The wooden shed has protruding nails, multiple areas of broken and splintered wood. The metal brackets are loose and protruding. b. There are widespread areas of peeling paint on the wooden railing that is used for entrance/exit from the playground. 2. The following safety hazards were observed inside of the facility: a. One of the mini blinds in classroom D has multiple areas that are broken and split apart. b. The plastic boarder is peeling away from the wall in the bathroom of classroom C. c. There is a broken area of floor tile outside of the bathroom of classroom F. d. The baseboards and bottom (inside) sink cabinet in the bathroom of classroom A have areas of dust, dirt and grime.
