Volunteers who work more than six hours per week shall receive training on the center's emergency procedures within the first week of volunteering. The file for Staff #1 did not contain documentation of a completed training on the center's emergency procedures as required.

The Center is required to ensure that children are always in the care of a staff member who has satisfactorily completed a training program for the purpose of administration of emergency medication that has been prescribed. There were no staff on site that were certified to administer Child #1?s medication.

Staff shall wash their hands with soap and running water, before and after helping a child use the toilet; before and after a diaper change; after the staff member uses the toilet; after any contact with body fluids; before feeding or helping children with feeding; and before preparing or serving food or beverages. Staff #2 did not wash their hands before changing a diaper, between the diaper changes of 2 children, or after diaper changing.

The Center is required to maintain documentation for staff records including two or more references as to character and reputation as well as competency were checked before employment. The file for Staff #1 (hired 2-19-26) did not contain 2 references.

The Center is required to keep a written record of daily attendance for each group of children documenting the arrival and departure of each child in care as it occurs. Evidence: During inspection in the Rainbow 1 classroom, there were 12 children present, but the written record reflected there were 16 children present.

The Center is required to keep hazardous substances in a locked place using a safe locking method that prevents access by children. Evidence: In the bathroom area where the door was unlocked, 3 containers of toothpaste containing labels that stated, ?Keep out of Reach of Children? and the word ?Warning? on the labels were stored on an open shelf.

(Repeat Violation) The Center is required to ensure that children under 10 years of age always be within actual sight and sound supervision of staff. Evidence: During inspection in the Rainbow 1 classroom, there were 2 teachers located on the far side of the classroom when a child exited the classroom through a gate and entered the hallway, then entered into the bathroom area behind another half wall where they were out of sight and sound supervision.

The Center is required to have at least one staff member in each classroom or area where children are present that had obtained CPR and first aid training. Evidence: The file for Staff #1 did not contain documentation for completion of CPR and first aid training.
