Staff members are required to obtain the results of a follow-up tuberculosis (TB) test or screening at least every two years from the date of the initial screening or testing. The subsequent TB screening required for Staff #1 was past due by more than five months.

The center is required to maintain a written record of daily attendance for each group of children that documents the arrival and departure of each child in care as it occurs. On 6/3/26 at approximately 12:40pm, four children were observed in the Infant classroom. The written attendance record contained documentation of three children present. In order to update the record, staff signed out one child who had moved to another class and signed in two who had arrived.

Staff are required to complete annually a minimum of 16 hours of training appropriate to the age of the children in care. The center was unable to provide documentation to demonstrate that Staff #1, Staff #3 and Staff #4 had completed 16 hours of training for the past year.

Lockdown procedures are required to be practiced at least annually. A lockdown drill was past due by more than one month.

The center may administer prescription medication provided the center has obtained written authorization from a parent or guardian. A prescription medication was observed onsite for Child A on 11/13/25. There was no written authorization from a parent or guardian available onsite for the medication.

When an authorization for medication expires, the center is required to notify the parent that the medication needs to be picked up within 14 days or the parent must renew the authorization. The medication authorization for Child B indicated that the authorizations needed to be updated prior to August 2025. The medication was observed onsite on 11/13/25 and the paperwork had not been updated.

The center is required to ensure that when they provide meals or snacks, that children three years of age or younger are not offered foods that are considered to be potential choking hazards. On 12/3/24, children 16-months-old through three-years-old were observed to be served hot dogs that were not cut lengthwise or in a manner to prevent possible choking.
