(Repeat Violation) For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs. During inspection in the Sunflower classroom (ages 16 months ? age 3), there were 7 children present, but the written record reflected there were 6 children present.

When staff are supervising children, they shall always ensure their care, protection, and guidance. On 4/8/26 when Child #1 (2 years old) left the preschool playground, Child #1 proceeded through a latched metal gate on the preschool playground and then an unlocked wooden gate surrounding the field. At the other end of the field, a gate exits to a parking lot. Staff reported that Child #1 has been trying to exit the playground from these gates for approximately 1 month, that staff have reported the gate lock is broken, and that there has been no resolution to keep an incident from occurring.

Children under 10 years of age always shall be within actual sight and sound supervision of staff. On 4/8/26, Child #1 left the preschool playground alone through 2 fence gates and was found by Staff #1 who saw them from inside the building on an adjacent field wandering around. Because the fence surrounding the field is solid wood, staff cannot see the field from the preschool playground. Child #1 was returned to the preschool class on the playground by Staff #1. The child was out of sight and sound supervision for approximately 5-15 minutes before anyone noticed.

The Center is required to ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition. On the older children?s playground, in the blue area by the shed, the poured in place ground cover has voids and peaks that are causing a tripping risk.
