COMPLAINT RELATED When staff are supervising children, they shall always ensure their care, protection, and guidance. On Wednesday, April 8, 2026, Child #1 approached Staff #1, crying, stating that another child had poked them in the eye. Staff #1 did not observe any signs of injury at that time. Staff #1 offered Child #1 a cold press for their eye and continued to ask the child throughout the day if they were okay. Staff #1 did not report the incident to the administration. Child #1 was seen by a medical provider and diagnosed with an abrasion of the left cornea. Staff #1 did not ensure their care, protection, and guidance by not reporting the incident after Child #1 continued to rub their eye. Complaint related: Yes

REPEAT VIOLATION The operational responsibilities of the licensee shall include ensuring that the center's activities, services, and facilities are maintained in compliance with these standards, the center's own policies and procedures that are required by these standards, and the terms of the current license issued by the department. The center did not follow its own policies and procedures, as required by the standards for identifying where children are at all times. On Friday, March 6, 2026, Child #1 (age 3) was left unsupervised in the bathroom for approximately five minutes during a group bathroom break. Staff #1 was unaware that the child had been left in the bathroom. Staff #1 reported that they did not take a headcount and did not have the CRS (Child Supervision Record) with them. The center?s procedures for identifying where children are at all times require staff to use a tool called the Child Supervision Record (CSR) used to track where children and teachers are throughout the day. A process called Name-to-Face is used to account for children, comparing each child?s name on the CSR to their face, confirming they are present.

The behavior guidance plan for parents states: "We will offer alternatives to children engaged in undesirable behavior by offering a different toy, suggesting a new activity, engaging the child in an activity with a teacher or another child, or by suggesting independent play. The employee handbook states, "Unacceptable child guidance is defined as certain discipline methods are simply not acceptable at KCE, including: Spanking, grabbing, pinching, or any physical punishment or restraint."

On 8/15/2025, Staff #1 stated that Child #1 (age: 4 years old) was throwing balls at them while on the playground. Staff #1 did not use appropriate behavior guidance that was constructive in nature, age and stage appropriate, and intended to redirect children to appropriate behavior.

The center's behavior guidance plan for parents states: "We will offer alternatives to children engaged in undesirable behavior by offering a different toy, suggesting a new activity, engaging the child in an activity with a teacher another child, or by suggesting independent play.
