R381-100-11(1)(a)-(g) — Provide and maintain active supervision

The Licensee was out of compliance with R381-100-11(1)(a) by failing to ensure that each caregiver provides and maintains active supervision of each child. During the inspection, the investigator observed that children were left unsupervised on the playground when their caregiver came into the center to get water. Inspection: Complaint, Investigation Inspection. Correction status: Manager approval, no follow up needed. Correction action: On May 21, 2025, the investigator verified that the Licensee would re-train staff on active supervision procedures including remaining physically present in the room or area with the children, as required in rule.

R381-100-13(7) — Plastic bags, latex gloves, and balloons inaccessible to under 5

The provider was out of compliance with this rule by having plastic bags large enough to fit a child's head accessible to children younger than five years old. In the Kinderprep room, there were plastic garbage bags on a shelf accessible to children. This was a repeat rule noncompliance. Inspection: Follow-Up Inspection. Correction status: Correction Verified. Correction action: On 12/30/2024, the licensor verified the provider made the plastic bags inaccessible as stated in rule.
