R381-100-11(1)(a)-(g) — Provide and maintain active supervision

The provider was out of compliance with Rule R381-100-11(1)(a) by not ensuring that caregivers provided and maintained active supervision of each child, including for children younger than five years old, remaining physically present in the room or area with the children. In the infant room, there was no caregiver physically present in the room where where an infant was sleeping in a crib. Inspection: Unannounced, Annual Inspection. Correction status: Correction Verified. Correction action: On 2/4/2026, the licensor verified that the director opened the gate so that there was an opening in which the caregivers and children could move freely, creating one room again, so the infant was then considered supervised as stated in rule.

R381-100-7(9)(a)-(g) — Caregiver requirements

The provider was out of compliance with rule R381-100-7(9)(c) by not ensuring that caregivers completed the 2-1/2 hours of preservice training offered by the department before caring for children. At the time of the inspection, there were two caregivers who had been caring for children at the facility who did not complete the 2-1/2 hours of preservice training offered by the department. Inspection: Unannounced, Annual Inspection. Correction status: Correction Verified. Correction action: On 3/28/2025, the licensor verified that the two caregivers completed the 2-1/2 hours of preservice training as stated in rule.
