    Then provider was out of compliance with R381-100-11(1)(c) by failing to ensure that each caregiver provided and maintained active supervision of each child including knowing the number of children in their care at any time. During an inspection, it was determined that a child was left unattended outside when the caregivers miscounted the number of children they had in their care when they came off of the playground and returned to their classroom. Inspection: Complaint, Investigation Inspection. Civil money penalty: $500. Correction status: Correction Verified. Correction action: On September 29, 2025, the licensor verified that the provider had re-trained staff to ensure every child is provided with active supervision, and that caregivers maintain active supervision, as required in rule.
