The name-to-face roll call was not completed when returning to the classroom in one classroom. — Visit: QRIS Monitoring visit,Technical Assistance — Action taken: A referral to CCR&R will be completed, and staff will be trained on name-to-face roll calls. The director spoke to the educator about completing the name-to-face roll calls. Also, management will conduct periodic monitoring and spot checks to ensure the procedure is being completed and documented at all required transition points throughout the day. Any discrepancies will be addressed immediately through coaching and retraining. Also, the staff will document every time they do the check.

The agency did not identify someone in the agency prior to letting them speak to a child. — Visit: Complaint Investigation,Technical Assistance — Action taken: The director spoke to the educators regarding checking IDs. Before anyone is allowed past the half door at the agency entrance and before they can talk to/ pick up any child they will need to check their ID. This needs to be done before anyone notifies the director that they are in the building. The director will also at this to their next staff meeting as a refresher.

A new educator did not complete the training requirements before working at the agency. — Visit: Complaint Investigation,Technical Assistance — Action taken: The educator was unable to return to work until she completed all required Pre-Service trainings. The agency will use a hire checklist to ensure all items are completed before a new educator is allowed to work at the agency.

The table used for eating in the infant/toddler room does not have safety restraints — Visit: Follow-up visit,Technical Assistance — Action taken: The table is no longer being used and has been removed from the classroom. The classroom now has a small round table and chairs, as well as high chairs, for the children to use during meals and snacks.

The agency will also do more room checks after meals.

Also, the director spoke to the educators about not being nervous when the LC is at the agency.
